These are all the forms required for enrollment. Must have the following prior to first day:
- Child Enrollment Form
- Photo Release Form
- Routine Trip Permission Form
- Child Release Form
- GHCC Waiver & Release Form
- Infant Information (only necessary for children 6 weeks to 18 months)
- Child Medical Statement (you have 30 days from enrollment to turn in along with a copy of shot records).
